Management of chronic kidney disease associated with type 2 diabetes in adults

✔️ Pharmacology of Finerenone:
  • Mechanism of Action: Finerenone is a nonsteroidal, selective antagonist of the mineralocorticoid receptor (MR). It blocks MR-mediated sodium reabsorption and MR overactivation in both epithelial (e.g., kidneys) and nonepithelial tissues (e.g., heart, blood vessels). This helps prevent fibrosis and inflammation.
  • Potency and Selectivity: Finerenone is highly potent and selective for MR, with no significant affinity for androgen, progesterone, estrogen, or glucocorticoid receptors.
  • Effects on Blood Pressure: In clinical studies, finerenone reduced mean systolic blood pressure by 3 mmHg and diastolic blood pressure by 1-2 mmHg within the first month of treatment. This reduction remained stable thereafter.
  • QT Interval: Finerenone does not prolong the QT interval at doses up to 4 times the maximum recommended dose.
  • Absorption: Finerenone is fully absorbed after oral administration, with a bioavailability of 44%. Peak plasma concentration (Cmax) is reached between 0.5 and 1.25 hours post-dosing. Food does not significantly affect its absorption.
